Is it very low fps in both windowed and full screen mode?
What version of Windows are you using? Are you using more than one monitor? What is the CPU and gfx card, and their usage % when playing? Is your soma folder on the exceptions list in your anti-virus? Lots of questions but helps narrow it down :)
