Once a character name has been used, it can't be used again. Even if the character has been deleted. I asked iSylver about this shortly after the server came out but can't remember the exact reason he gave. It is however, intended.

I think it's so of someone accidently deletes a character it saves it for restoration purposes. I intended for my mage to be called adathon but I already used it on a noob so made adath instead.

If a character is deleted he is not really removed completely, just stored in a big list.
All the items are also stored with the name. I'm sure once it's "deleted" if you try to restore you gain all items back.
