Because you're doing it completely wrong.

It is working as intended, it is hitting all adjacent monsters.

You are casting on one monster, and it is hitting the 3 other monsters directly in contact with that monster.

That is why you are hitting 4.

You need them all to be standing directly next to the one which you are casting on.

Must be solely confined to dungeon, as I have just tested it with iSylver and it was working perfectly fine.

Literally about 10minutes ago.

its working fine now, you gotta stand in the middle of them to hit them all, i was standing at the side like you usually have to but that doesnt work anymore. in middle you hit 5 at side u hit 4 its just a tiny bit more work :p

It's still bugged in the dungeon. Can't Aoe properly solo or using another char as a target, it's fine however in other maps like Infernal Caves.

Tested with 5th knuckle.
Also doesn't hit more than 5 when in dungeon.
